Membership Terms

I have read and understand all of the following:

   •    The credit card I provide will be billed monthly. If that card fails to charge, my backup payment method will be charged.

   •    My membership is non-transferable, and I will be asked to present a government-issued photo I.D. to redeem membership benefits.

   •    To cancel my membership, I must submit an online request by the 25th of the current month. Cancellation requests may not be made via call or text.

   •    I may skip a payment for up to two months during my membership. After that, skipping additional payments is not allowed.

   •    Membership service benefits or rollover service benefits may be used for a lesser or equally priced service, but do not hold any cash value and may not be redeemed for multiple services with lesser or equal cash value.

   •    Accrued membership service benefits will not be available if the membership payment is past due or canceled.

   •    If I cancel my membership, any benefits I have already paid for will remain available for one year from the cancellation date. I may still use these benefits, but I must pay the difference between the membership price and the full service price. There are no exceptions.

   •    Unused or accrued membership service benefits are ineligible for refunds.

   •    No call/no show appointments are subject to forfeiture of the membership benefit for the scheduled service.

   •    All appointments are subject to availability, and it is strongly recommended that I schedule appointments in advance.
